We are searching for a talented Community Health Worker I

 

Hours: 40hrs

Shift:  Day shift; 8:00am - 4:30pm

Location: Southcoast Business Center – Fairhaven, MA; Position will follow a hybrid schedule

Responsibilities

Position reports to the Director, Manager, Team Lead or designee, in collaboration with a multidisciplinary care team, provide culturally appropriate health education, coaching, support, information and outreach in alignment with care plan in a variety of settings including community-based, primary care, ambulatory, emergency and acute care settings. Identify high risk patients utilizing effective listening and communication skills to address health and social needs. Utilize critical thinking to navigate systems and facilitate the coordination of care and community resources to meet the patient�s plan of care. Provide culturally appropriate health education, and outreach in a variety of community and health care settings. Implement culturally tailored interventions to meet patients short-term and long-term health and social needs.

Qualifications

  • Equal to completion of 4 years of high school plus additional specialized courses or training as required by role.
  • Two years direct experience in community-based outreach.
  • Proven ability for independent decision making and problem solving while performing effectively as part of a multi-disciplinary team.
  • Solid knowledge of community resources and effective teaching/coaching skills.
  • Bilingual skills preferred.
  • CHW certification preferred.
  • Verifiable good driving record and reliable transportation.
  • Community Health Worker preferred.
  • Massachusetts Vehicle Operators Class B license is required
  • . A clear DMV driving record is also required.
  • Must be fully vaccinated against seasonal Influenza and the COVID 19 virus or to be exempt from the requirement for medical or personal reasons by signing a statement certifying you are choosing to be exempt from vaccination once hired. 

     

  • Compensation: Pay rate will be determined based on level of experience. 

Pay Range

USD $21.84 - USD $36.72 /Hr.
