Vice President of Southcoast Health at Home
Reports To: Executive Vice President, Chief Operating Officer

 

Position Summary:

 

The Vice President of Southcoast Health at Home is responsible for the strategic, operational, and business leadership of Southcoast Health’s Home Health, Palliative Care, and Hospice services. This executive plays a key role in driving performance, ensuring regulatory compliance, enhancing care delivery, and advancing growth initiatives across the division. This is a board facing executive managing board expectations.

 

The Vice President collaborates closely with senior leaders across Southcoast Health to support the organization’s mission and maintain a strong commitment to high-quality, efficient, and patient-centered care.

 

Three Directors report directly to this position:

  • Director of Compliance, Quality, and Education
  • Director of Supportive Care (Hospice and Palliative Care)
  • Director of Telehealth and Transitional Care

This role also involves frequent collaboration with system-wide leaders and stakeholders to integrate and align Southcoast Health at Home with broader organizational strategies. Key partners include the SVP Chief Clinical Officer, Vice President of Population Health, Director of Case Coordination, Vice President of Care Center Operations, Vice President of Revenue, and Vice President of Clinical Operations.

 

Responsibilities

Strategic Leadership

  • Develop and execute strategic initiatives to expand Home Health, Palliative, and Hospice services in alignment with Southcoast Health’s mission and goals.
  • Monitor healthcare trends and identify market opportunities to support sustainable growth and innovation.

Operational Management

  • Oversee daily operations to ensure high-quality, efficient, and compliant care delivery.
  • Implement and maintain policies, procedures, and workflows that support operational excellence.
  • Monitor financial performance, and manage resources effectively.

Clinical Oversight

  • Ensure full compliance with Medicare, Joint Commission, and other applicable federal, state, and local regulations.
  • Collaborate with clinical leadership to improve patient outcomes, ensure safety, and reduce readmissions across care settings.

Leadership & Team Development

  • Provide direction and mentorship to a leadership team, including three directors overseeing clinical and administrative staff.
  • Cultivate a collaborative, accountable, and mission-driven team culture.
  • Promote staff engagement, leadership development, and succession planning.

Performance Metrics & Quality Improvement

  • Track and analyze key performance indicators (KPIs), including quality outcomes, patient satisfaction, and operational efficiency.
  • Lead continuous improvement initiatives using data-driven approaches to optimize care delivery and patient experience.

Board Engagement

  • Prepare and lead monthly board meetings in accordance with Southcoast Health at Home bylaws.
  • Communicate when needed with board members, providing updates, reports, and strategic insights.

Stakeholder Engagement

  • Serve as a liaison to physicians, referral sources, regulatory bodies, and community partners.
  • Represent Southcoast Health at Home in community forums, public relations activities, and professional organizations to build awareness and partnerships.

Qualifications

  • Bachelor’s degree in Nursing, Healthcare Administration, or a related field (Master’s degree strongly preferred).
  • Active RN license (preferred, depending on clinical responsibilities).
  • Minimum of 8 years of progressive leadership experience in Home Health or Post-Acute Care.
  • Strong knowledge of CMS regulations, value-based care models, and risk-based contracting (e.g., ACOs, TEAMS).
  • Proven record of success in clinical and operational leadership within a healthcare setting.
  • Exceptional leadership, interpersonal, communication, and organizational skills.
  • High degree of business acumen and financial management expertise.

Preferred Skills:

  • Experience with EMR platforms (e.g., Homecare Homebase, Epic).
  • Lean Six Sigma or equivalent process improvement certification.
  • Knowledge of PDGM (Patient-Driven Groupings Model) and Value-Based Purchasing (VBP) programs.
