We are searching for a talented Community Health Worker-Behavioral Health

 

Hours: 40hrs

Shift:   Day shift; 7:00am - 3:30pm

Location: St. Luke’s Hospital – New Bedford, MA

Responsibilities

Position report to the Chair of Psychiatric Services or designee. Work as a member of a multi-disciplinary psychiatric CL team, and in collaboration with Medical Services, to promote patient autonomy and empowerment, address SDOH barriers to support treatment recommendations and optimal disposition for patients with a range of behavioral health concerns. Facilitate the coordination of community resources to meet the patient�s plan of care. Assist patients to access necessary social, medical, and behavioral support services and programs.

Qualifications

  • Equal to completion of 4 years of high school plus the completion of additional courses/training in community health work is required.
  • Community Health Worker certification preferred.
  • Must be culturally competent and have thorough knowledge of the communities being served.
  • Two years� experience working with behavioral health patients.
  • Proven ability for independent decision making, problem solving, and effectively utilize supervision while performing as part of the multi-disciplinary team.
  • Solid knowledge of community resources and effective relapse prevention/recovery skills.
  • Must have basic competency with computer skills.
  • Experience with Outlook and EMR is preferred. Bilingual skills preferred.
  • Community Health Worker certificate preferred.
  • Must be fully vaccinated against seasonal Influenza and the COVID 19 virus or to be exempt from the requirement for medical or personal reasons by signing a statement certifying you are choosing to be exempt from vaccination once hired. 

     

  • Compensation: Pay rate will be determined based on level of experience. 

Pay Range

USD $21.84 - USD $36.72 /Hr.
