Hours:  40 hrs 8:00A – 4:30P Driving required.

Shift:   Day – no weekend, holiday or call.

Responsibilities

  • Under Direction of the Director of Transitional Care and in collaboration with the Physician, provide an advanced level of nursing practice to deliver health care services to patients and families.
  • Provide advanced clinical assessments, diagnoses, and treatment plans for high-risk patients receiving care at home.
  • Practice independently and in collaboration with appropriate physicians to provide primary care to patients and oversee conduct and or complete all required visits and documentation in accordance with CMS regulations for face -to face encounter visits.
  • Supervise, mentor, and coordinate a multidisciplinary team of nurses and health professionals.
  • Ensure effective communication and collaboration across the care team.
  • Oversee and optimize care plans for high-risk patients with complex medical needs, ensuring continuity of care, timely interventions, and reduced hospital readmissions.
  • Lead implementation and ongoing management of RPM program, ensuring best practices, virtual visits, and clinical documentation.
  • Monitor patient outcomes, conduct case reviews, and lead initiatives to improve care quality, patient satisfaction, and adherence to regulatory standards.
  • Ensure the team adheres to all relevant federal, state, and organizational regulations and guidelines for home health and RPM services.

Qualifications

  • Nurse Practitioner: Master of Science Degree in Nursing is required.
  • Certification from a National accrediting body to practice as a Nurse Practitioner is required.
  • Current Massachusetts R.N. registration is required.
  • Current Massachusetts and RI RN license registration is required.
  • Nurse Practitioner certificate is required.
  • Proven ability to care for adult patients with a broad range of physical and psychological problems is required.
  • Proven ability to demonstrate maturity, self-confidence, and the ability to follow complex patient management plans as directed by the attending physician is required.
  • Qualified in all basic and advanced cardiac support standards is required.
  • Nurse Practitioner: Current license to practice in the Commonwealth of Massachusetts as a Registered Nurse.
  • Certification (of current eligibility) issues by the Commonwealth of MA and the State of RI to practice as a registered nurse is required.
  • Current Nurse Practitioner license is required.
  • Must possess current CPR certification.
