We are searching for a talented Team Lead - Telehealth Program - RN

 

Hours: Full Time

Shift: Day shift, 8:00am - 4:30pm   

Location: Visiting Nurse Association - Fairhaven, MA

Compensation: Pay rate will be determined based on level of experience. 

 

Responsibilities

Under general supervision of the Director, Telehealth and Transitional Care or designee, perform duties to support and in support of the Tele monitoring program in an effort to reduce re-hospitalizations and provide proactive symptom management while adhering to best practices. Duties include, but are not limited to: address data packets that are sent via the telehealth system with patients, field staff, physicians and clinic staff; triage phone calls and provide patient education as it relates to data collected via telephone and video as needed; troubleshoot equipment and make home visits as needed. Implement and continue to grow programs to reduce re hospitalizations amongst high risk patients.

Qualifications

  • Current Licensure by Massachusetts Board of Registration in Nursing Requires (RN).
  • Proficient computer skills, including experience with various software, database and electronic records programs is required.
  • Two (2) years of related work experience is required; recent home care experience is preferred.
  • Current RN Licensure is required in both Rhode Island and in the Commonwealth of Massachusetts.
  • Basic Life Support (BLS) & CPR certification is also required.
  • Must be fully vaccinated against seasonal Influenza and the COVID 19 virus or to be exempt from the requirement for medical or personal reasons by signing a statement certifying you are choosing to be exempt from vaccination once hired

Pay Range

USD $40.74 - USD $72.46 /Hr.
