We are searching for a talented Social Worker-Clin-Case Coordinator-LICSW

 

*$1,800 new hire sign-on bonus with LICSW or LMHC license at time of hire. (rehires termed more than 1 year also eligible)*

 

Hours: 24hrs

Shift:  Day/Evening shift with weekend and at least one holiday per rotation

Location:  Charlton Memorial Hospital - Fall River, MA

Compensation: Pay rate will be determined based on level of experience. 

 

Responsibilities

Under the direction of the Manager, Clinical Social Work Services; perform diversified duties both within and in support of the SHG Care Coordination functions. Provide case management services including but not limited to the application of clinical diagnostic, assessment or treatment knowledge or skills. Identify and address psychosocial factors that may impede the patients' ability to optimize benefit from the health care delivery experience. Coordinate the care/services of assigned patients along the continuum of care. Participate in discharge planning to assure timely individual patient discharges within the scope of position responsibilities. Facilitate patient/family/representative achievement of desired outcomes. Responsibilities may include on call/shift coverage and holiday coverage.

Qualifications

  • A Masters Degree in Social Work or related field from an accredited college or university is required or the equivalent knowledge and experience.
  • Knowledge of funding, resources, services, clinical standards and outcomes is required.
  • Current Massachusetts Social Work license at a minimum of LICSW is required.
  • A minimum of 5 years related work experience including one-year in a medically oriented setting is required.
  • One year case management experience preferred.
  • Must possess strong creative problem solving, critical thinking, time management and organizational planning skills as well as multitask skills and the ability to tolerate interruptions and a demanding work assignment.
  • Computer skills are essential.
  • Certification in Case Management is preferred.
  • Must be fully vaccinated against seasonal Influenza and the COVID 19 virus or to be exempt from the requirement for medical or personal reasons by signing a statement certifying you are choosing to be exempt from vaccination once hired. 

Pay Range

USD $60,403.00 - USD $103,459.00 /Yr.

Rotation Schedule Requirement
