Compensation: Pay rate will be determined based on level of experience. 

 

Responsibilities

Under the supervision of the Executive Director, or designee, perform diversified duties related to ongoing review of activities related to utilization of resources for the provision of a Care Management Program, including transitions of care, chronic and complex care management services, delegated case management, integration with and support of patient centered medical homes, and support of chronic disease management programs for high risk and at risk populations. Coordinate provision of evidence-based, cost-effective case management services related to all aspects of population health management. Work with Executive Director to develop and maintain an evidence-based, highly effective care management team and practice model; maintain strong relationships with Network physicians. Provide on-going direction and supervision of personnel assigned to the Southcoast care management populations. Maintain a patient caseload as needed to adequately staff the department. Will oversee new OneCare and SCO delegation. Will build program, standard work and team in collaboration with the Executive Director.  

Qualifications

  • Bachelor's Degree in Nursing or equivalent is required. Case management certification is preferred.
  • Duties require expertise in medical terminology, healthcare delivery systems and applicable legal, regulatory and accreditation requirements for both federal and Massachusetts- specific requirements.
  • Experience in population health case management/care coordination is preferred; 5 years of related work experience including progressive and innovative leadership responsibilities are required.
  • Excellent interpersonal skills.
  • A licensed RN in both the Commonwealth of Massachusetts and the State of Rhode Island is required.
  • Must be fully vaccinated against seasonal Influenza and the COVID 19 virus or to be exempt from the requirement for medical or personal reasons by signing a statement certifying you are choosing to be exempt from vaccination once hired. 

Pay Range

USD $97,864.00 - USD $179,400.00 /Yr.
