We are searching for a talented Social Worker-Clinical

 

Hours: Per Diem

Shift:  Day/Evening shift with weekend and holiday rotation; 8:00am - 4:30pm and 3:30pm - 12:00am

Location: St. Luke's Hospital - New Bedford, MA; this is a hybrid schedule

**Available to regular status employees who are scheduled to work a minimum of 24 hours.

Responsibilities

Under the direction of the Manager, Clinical Social Work Services; perform diversified duties both within and in support of the social work functions. Under the supervision of an LICSW, provide case management services independently including but not limited to application of clinical diagnostic, assessment or treatment knowledge or skills. Identify and address psychosocial factors that may impede the patients' ability to optimize benefit from the health care delivery experience. Intervene as necessary preventing over and under utilization of health care resources. Actively participate in discharge planning to assure timely individual patient discharges within the scope of position responsibilities.

Qualifications

  • A Masters Degree in Social Work from an accredited college or university or the equivalent knowledge or experience is required.
  • Current Massachusetts Social Work license at a minimum of a LCSW is required.
  • Over three (3) years of related work experience including one (1) year experience in a medically oriented setting is required.
  • Computer skills are essential.
  • A minimum of LCSW license in the Commonwealth of Massachusetts is required. Certification in Case Management is preferred.
  • Must be fully vaccinated against seasonal Influenza and the COVID 19 virus or to be exempt from the requirement for medical or personal reasons by signing a statement certifying you are choosing to be exempt from vaccination once hired. 

Pay Range

USD $27.13 - USD $45.60 /Hr.
